How to order a repeat prescription

You are now able to request repeat prescriptions by email. Please click here for further details. Otherwise please follow these guidelines:
 
Please hand your prescription to your nominated Lloyds pharmacy (either Lloyds on the High Street or Lloyds in Victoria Square opposite the surgery).  They will then forward the request to us having logged it. The prescription should be ready to collect 48hrs later.

The first time you hand in your repeat prescription at Lloyds pharmacy you will be required to complete a short form. Further information regarding this is available from the Lloyds pharmacy website.

 If you need a repeat prescription then you will need to hand in your request 48 hours before you need more tablets. In most cases you will have a B-side to the prescription (see below) and merely need to tick the appropriate drug required. 

 If you require a drug that is not on the B-side of your prescription then you will need to fill in a repeat medication form (available at both Lloyds pharmacies and the surgery). Please remember to fill in your full details so that we know who wants it!

 Telephone requests cannot be taken unless you are housebound or it is an emergency.

If you would like us to post it we need a stamp. Some local chemists will collect but we do not arrange it.
